The work
Grinding is priced by the width of the stump. Around this county that generally works out between $5 and $12 per inch of stump diameter, with a minimum charge for a single small stump because the machine still has to be brought out.
Typically a few inches to about a foot below grade, which is enough for sod, a bed or a post. If something with a footing is going in, say so and we will go deeper or recommend full excavation instead.
Not from a ground stump. The remaining root system is dead and decays in place over several years. A few species send up suckers from roots afterwards, which is a mowing problem rather than a returning tree.
Not straight into fresh chips, which tie up nitrogen while they break down. Either dig the chips out and backfill with soil, or plant the replacement a few feet to one side of the old stump.
Your choice. We can rake the chips back in, which settles over a season, or haul them off and leave a clean hole for topsoil. Tell us which when we quote so we bring the right truck.

The township threshold concerns removing live trees, not grinding a stump that is already cut. If the tree is still standing, that part of the job is the part with the paperwork.
