The work
Grinding is priced off the width of the stump, measured across the cut face. In Morris County that lands between $5 and $12 an inch. One small stump on its own carries a floor price, since the grinder has to be trailered out either way.
Somewhere between a few inches and a foot under grade, which covers sod, a planting bed or a fence post. Anything with a footing under it is a different job: tell us up front and we either go deeper or advise digging the stump out whole.
Not from a stump that has been ground. What is left of the root system dies and breaks down underground over a few years. Some species do push suckers up off those roots, but that is something the mower handles, not a tree coming back.
Not into fresh grindings. They pull nitrogen out of the soil while they rot down. Scoop the chips out and backfill with topsoil, or set the new tree a few feet off to the side of where the old one stood.
However you want it. The grindings can go back in the hole and settle over a season, or they leave with us and you get a clean void ready for topsoil. Say which at the quote so the right truck turns up.

Always, before the wheel touches anything. On lots this age there are irrigation lines, invisible fencing and old service runs that nobody has a drawing for any more.
Most of the time, yes. A walk-behind machine clears the majority of side gates. Measure the tightest point on the route for us, steps included, and you will get a straight answer on whether it fits.
Not automatically. They are separate operations with separate machines, so they are quoted separately and you can take one without the other.
Yes, and plenty of people do. They make reasonable bed mulch. Just keep them off the crown of anything you are trying to grow and do not plant into them.
The township threshold concerns removing live trees, not grinding a stump that is already cut. If the tree is still standing, that part of the job is the part with the paperwork.
